Here you can access the client portal and fill out your registration forms. If you need to fill out the registration form at our office, you are more than welcome to do so. It is recommended that you arrive 15min before your first appointment to fill out the form form.<\/p>\n

As a current client, you have direct access to the Client Portal. Your personal PIN is provided by your clinician at your first session; if you don\u2019t recall it, a new one can be easily generated for you.<\/p>\n
